Under the supervision of a chemical dependency professional as defined by the Department of Health, implements, maintains and coordinates the case management activities of assigned patients.  Delivers treatment services that include individual and group counseling related to chemical dependency patients.  Establishes and maintains patient files to WAC and CARF standards and NWITC policies.  Participates in quality assurance activities, educational lectures, coordinates case management, completes discharge summary, makes appropriate aftercare plans and communicates with appropriate referring and aftercare organizations.  Maintains time lines as indicated by NWITC policies.  Coordinate inter-agency and inter-department patient management.  Contribute to positive, effective development by cooperating with other staff to implement a consistent treatment plan for each patient across shifts and in the absence of the primary counselor or counselor-trainee. The successful trainee II is able to respond to a variety of physical symptoms and behaviors with strategic and effective treatment plans and interventions, and able to manage patient group activities therapeutically.

1.                   Provides case management for an assigned caseload of patients per NWITC policies and procedures, DOH WACs and CARF standards.

2.                   Completes intake and assessment on each new patient within ten days of admission.  Communicate essential dynamics, background information to other staff members.  Communicate current medical needs to the nurse.

3.                   Maintains accurate and timely documentation in patient records as per NWITC policies and procedures.

3.             Facilitates patient group therapy sessions.

4.             Arranges and participates in family, significant other conferences as appropriate.

5.             Initiates and implements phone calls, correspondence with patients, referral sources, probation officers, after care providers, attorneys and others as appropriate.

6.             Monitors patient milieu to include behavior, participation, health and safety; interacts and intervenes with patients therapeutically and consistent with the treatment plan.

7.             Maintains confidentiality regarding patient/client information.

8.             Attends weekly staffing meetings, staff meetings and in services as required or that occur on the assigned shift.

9.             Cooperates with staffing patterns and scheduling needs for meeting patient needs; participates in temporary assignments.

10.          Provides pre-admission and admission assessments.

11.          Monitors and maintains current data for outcomes as assigned.

12.          Interacts cooperatively and constructively and with timeliness with referring agencies and aftercare agencies and others with patient involvement.

13.          Transports patients.  Picks up supplies and resources as needed.

14.          Participates in the development of the lecture manual.

15.          Informs director or clinical coordinator of any problems potential or actual that affect safety or health of patients or staff.

17.                Maintains orderly office, coffee and supply area.

18.                Participates in quality assurance activities using appropriate forms, collects data, makes reports and corrects problems. Participates in data collection TARGET system, equivalent or substitute, enters into computer.

19.          Interacts positively with peers to build a cooperative team and enhance morale.  Communicate respectfully and professionally modeling effective, responsible style for patients.

20.          Interacts with all patients of the milieu as needed.

22.          Completes intakes and assessments as requested.

23.          Maintains a written record of counseling activities and time for the purpose of becoming a chemical dependency counselor.

24.          Enrolls in and completes courses (relevant to counselor certification and approved by NWITC).

25.          Responds to the direction and requests of the intern supervisor regarding assignments, request for information or copies of academic courses.

26.          Studies counseling theories and techniques as assigned by the intern supervisor; becomes able to use techniques as appropriate. 

This position has no supervisory responsibilities.

Must be certified as a Chemical Dependency Professional Trainee by the State of Washington. Must not be under current investigation by the Department of Health or found in noncompliance with DOH ethical standards unless successfully participating in a correction program with DOH and the Squaxin Island Tribe is in agreeance.

High School Diploma or GED and two years of relevant experience plus one year of chemical dependency trainee experience or one year of case management activities. Experience working with Native Americans is preferred. 

Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.

Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference and volume. Ability to apply concepts of basic algebra and geometry. 

Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. 

Must have a current, valid Washington State Driver’s License.  The successful applicant must pass a criminal history background check and must not have been convicted of a felony within the past two years.  Must be insurable by the Tribe’s insurance carrier. Must complete TB test, CPR and HIV/Aids training course.

While performing the duties of this job, the employee is occasionally exposed to outdoor weather conditions. The noise of the work environment for this position is usually moderate.

Knowledge of industry recommended cleaning methods is desirable. The job requires the ability to work independently with limited supervision as well as part of a team.  Regular and consistent attendance is a condition of employment.  The employee must deal courteously with public, patients, and staff. Applicants who have been a patient of the NWITC programs must have completed services for a minimum of six months and should have six months of sustained recovery.

The successful candidate will be required to have a urinalysis for drug and alcohol screening in accordance with the Tribe’s Drug Free Workplace Policy and falls within the higher level of random testing as defined by the policy.

Indian Preference will be exercised in the hiring of this position in accordance with the Tribe’s Personnel Policies.
