Summary:
Provide treatment to the Squaxin Island Tribal community, coordinating treatment services to patients with dual diagnosis disorders.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
(Other duties may be assigned)

  • Provide case management for an assigned caseload of patients per SIT policies and procedures, and DASA WAC’s.
  • Maintain accurate and timely documentation in patient records as per SIT policies and procedures.
  • Facilitate patient group therapy sessions.
  • Arrange and participate in family, significant other conferences as appropriate.
  • Initiate and implement phone calls, correspondence, progress reports with patients, referral sources, probation officers, after care providers, attorneys and others as appropriate.
  • Maintain confidentiality regarding patient/client information.
  • Attend weekly staff meetings and in-service as required.
  • Cooperate with staffing patterns and scheduling needs for meeting patient needs; participate in temporary assignments.
  • Provide pre-admission and admission assessments.
  • Monitor and maintain current data for out comes as assigned.  Provides data and reports required for successful billing, collect necessary information to maximize billing opportunities.
  • Interact cooperatively and constructively and with timeliness with referring agencies and after care agencies and others with patient involvement.
  • Transport patients. Pick up supplies and resources as needed.
  • Participate in the development of the lecture manual.
  • Inform supervisor of any problems potential or actual that affect safety or health of patients or staff.
  • Maintain orderly office, coffee and supply area.
  • Participate in quality assurance activities using appropriate forms, collect data, make reports and correct problems.
  • Participate in data collection TARGET system, equivalent or substitute, enter into computer.
  • Interact positively with peers to build a cooperative team and enhance morale. Communicate respectfully and professionally, modeling effective, responsible style for patients.
  • Complete intakes and assessments as requested
  • Accept assignments with the treatment program as requested.
  • Other duties as assigned.

SUPERVISORY RESPONSIBILITIES:
This position has no supervisory responsibilities.

QUALIFICATIONS:

EDUCATION and/or EXPERIENCE:
Bachelor’s Degree in health, human service area or related field, and one year experience as a licensed and certified chemical dependency treatment counselor OR an Associates Degree in health, human service area or related field and three years experience as a licensed and certified chemical dependency treatment counselor.

LANGUAGE SKILLS:
Ability to read, analyze and interpret common scientific and technical journals, financial reports, and legal documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to write, speeches and articles for publication that conform to a prescribed style and format. Ability to effectively present information to top management, public groups, and/or boards of directors.

MATHEMATICAL SKILLS:
Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference and volume. Ability to apply concepts of basic algebra and geometry.

REASONING ABILITY:
Ability to define problems, collect data, establish facts and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.

CERTIFICATES, LICENSES, REGISTRATIONS:
Must be a chemical dependency professional licensed by the Department of Health. A valid Washington State driver’s License and proof of insurance; insurable by the Tribe’s insurance program (and able to transport patients).  TB Test, and current CPR and first aid training certification required.
