Summary:
As a professional-level counselor, implements, maintains and coordinates the case management activities of a patient load.  Delivers treatment services that include individual and group counseling related to all patients including the more difficult chemical dependency patients. Establishes and maintains patient files to RCW, WAC, CARF, NWITC policies. Maintains time lines as indicated by NWITC policies. Coordinates services for patients with dual diagnosis.

ESSENTIAL DUTIES AND RESPONSIBILITIES: Include the following;

  • Provides case management for an assigned caseload of patients per NWITC policies and procedures, DOH and RCW’s, WAC’s, CARF standards and organization guidelines.
  • Works as a team to facilitate ASIST trainings to employees and tribal communities to help them develop skills in suicide prevention.
  • Regularly reviews open records to assure that timelines are met for documentation.
  • Maintains accurate and timely documentation in patient records as per NWITC policies and procedures.
  • Facilitates patient group therapy sessions.
  • Arranges and participates in family, significant other conferences as appropriate.
  • Initiates and implements phone calls, correspondence, progress reports with patients, referral sources, probation officers, after care providers, attorneys and others as appropriate.
  • Monitors patient milieu to include behavior, participation, health and safety; interacts and intervenes with patients therapeutically and reviews the consistency of care with the treatment plan.
  • Maintains confidentiality regarding patient/client information.
  • Attends weekly staff meetings and in-service as required or that occur on the assigned shift.
  • Cooperates with staffing patterns and scheduling needs for meeting patient needs; participates in temporary assignments.
  • Provides pre-admission and admission assessments. Develops treatment plans that target resolution of trauma, expression of grief and loss, development of positive identity.
  • Provides treatment to assist patients internalize skills and establish priorities to support sustained recovery.
  • Monitors and maintains current data for out comes as assigned. Provides data and reports required for successful billing, collects necessary information to maximize billing opportunities.
  • Interacts cooperatively and constructively and with timeliness with referring agencies and after care agencies and others with patient involvement.
  • Transports patients. Picks up supplies and resources as needed.
  • Participates in the development of the lecture manual. Assists in the development of model typical treatment plans addressing patterns of patient needs.  Assists in the development of lecture material, handout material and other resources to improve treatment outcomes, meet patient needs and that are responsive to changing patterns of patient characteristics.
  • Assists with the preparation for all external agency requirements including, DOH, IHS and CARF.
  • Informs director or clinical coordinator of any problems potential or actual that affect safety or health of patients or staff.
  • Maintains orderly office, coffee and supply area.
  • Participates in quality assurance activities using appropriate forms, collects data, makes reports and corrects problems
  • Participates in data collection TARGET system, equivalent or substitute, enters into computer
  • Interacts positively with peers to build a cooperative team and enhance morale. Communicate respectfully and professionally, modeling effective, responsible style for patients
  • Interacts with all patients of the milieu as needed.
  • Participates in the pre-admission process for prospective patients as requested
  • Completes intakes and assessments as requested

SUPERVISORY RESPONSIBILITIES:
This position has no supervisory responsibilities.

QUALIFICATIONS:
Must be certified as a Chemical Dependency Professional by the State of Washington. Must not be under current investigation by the Department of Health or found in noncompliance with DOH ethical standards unless successfully participating in a correction program with DOH and the Squaxin Island Tribe is in agreeance. Experience working in an area of human services with Native American patients preferred.

EDUCATION and/or EXPERIENCE:
Bachelor’s Degree in health, human service area or related field, and one-year experience as a licensed and certified chemical dependency treatment counselor OR an Associate’s Degree in health, human service area or related field and three years’ experience as a licensed and certified chemical dependency treatment counselor.

LANGUAGE SKILLS:
Ability to read, analyze and interpret common scientific and technical journals, financial reports, and legal documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to write, speeches and articles for publication that conform to a prescribed style and format. Ability to effectively present information to top management, public groups, and/or boards of directors.

MATHEMATICAL SKILLS:
Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference and volume. Ability to apply concepts of basic algebra and geometry.

REASONING ABILITY:
Ability to define problems, collect data, establish facts and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.

CERTIFICATES, LICENSES, REGISTRATIONS:
Must have a current, valid Washington State Driver’s License.  The successful applicant must pass a criminal history background check and must not have been convicted of a felony within the past five years.  Must be insured by the Tribe’s insurance carrier. Must complete TB test, CPR and HIV/Aids training course.

While performing the duties of this job, the employee is occasionally exposed to outdoor weather conditions. The noise of the work environment for this position is usually moderate.

OTHER:
The job requires the ability to work independently with limited supervision as well as part of a team.  Regular and consistent attendance is a condition of employment. The employee must deal courteously with public, patients, and staff. Applicants should have three years of sustained recovery. TB test, HIV/AIDS training, CPR and First Aid course required.

DRUG FREE WORKPLACE:
The successful candidate will be required to have a urinalysis for drug and alcohol screening and stay in compliance with the Tribe’s Drug Free Workplace Policy. This position falls within the higher level of random testing as defined by the policy.

Indian Preference will be exercised in the hiring of this position in accordance with the Tribe’s Personnel Policies.
